One of life’s major pivot points is what to do after high school.  How to make the change from high school student to whichever path we want to pursue? 

Our Post High School Transition program focuses on helping at-risk youth and those with disabilities transition smoothly from high school to the military, full-time employment, the trades, or post-secondary education.

In coordination with the local school districts, our Post High School Transition program develops an individualized plan for participating students. These individualized plans are based on assessments and the chosen level of “Person Centered Planning”

Program Components Include:

Armor of Resilience

A six-week course that teaches the how to take control of your life. We talk about how resilience is like armor and how it can protect us.

What is resilience? It’s the ability to withstand adversity and bounce back from difficult life events.

Person Centered Planning

A goal-setting process that focuses on the positive traits of a person and helping them reach the life they want. We look at your strengths, who can support you, concerns, and help you with action steps. Anyone can call and make an appointment to learn more and get a free evaluation.

    There are three different levels you can choose:

    Focuses on 2 to 4 areas you want to improve.
    Map
    Focuses on one goal.
    Path
    The big dream. What do you want your future to be?

    Accessing Education and/or Employment Services

     If you’re a student with a learning or physical disability, there are many college resources & services to help you with homework, studying, and navigating daily life.  Equally, employers also provide accommodation services to help their employees with disabilities perform their duties to the best of their abilities. 

    Together with either your college, trade school and/or employer, we’ll help you navigate which student or employee services best fit your needs and circumstances.  

    Developing Job & Soft Skills

    Participants will receive instruction on basic, universal job skills found in nearly every industry and type of employment.  These will also include a variety of soft skills such as

    Learning How to Self-ID

    Self-identity is how you identify & define yourself base on your perception of specific traits, abilities, qualities & charactersitics that you feel represent you. Becoming aware of your self-id will help you make better decisions now and in the future. Our Post High School Transition program will provide the tools & resources to help you identify & understand your Self-ID.

    Gardianship . . .?

    What is “Guardianship” and why might you consider it for your at-risk or disabled youth?  We’ll help you answer these questions and explore the benefits & disadvantages (pros & cons) of guardianships.
